New Patient Process
1. Register your child using the PATIENT PORTAL link at the top of this page. Once registered and scheduled, their appointments will show up on their dashboard where you will complete the SELF CHECK-IN process a few days before your appointment. This is done to remain COVID friendly and minimize your time in the waiting room completing forms.
2. During Self Check-In, please complete the family, social and medical history as well as our COVID screening questionnaire. Once those are complete, you will electronically sign our Office and Financial Policy, Notice of HIPAA Privacy and Acknowledgement of Office Policies.
3. When you arrive for your child’s appointment, please have your license and insurance card ready for us to verify and scan in to our system. We will have you complete two consent forms for treatment and release of records if needed prior to the start of your visit. Paper copies of all of our forms are listed below should you need them for reference. We look forward to seeing you!
