Insurance Basics

Every insurance carrier and plan is different! Click on the link below for helpful insurance information and explanations of common insurance terminology.

Newborns MUST be added to your plan within the first 30 days of life. Unfortunately, insurance companies do not offer any exceptions to this timeline. Please call your insurance company (or HR if it applies) ASAP to add them to your plan. Failure to do so will contribute to unnecessary and potentially large bills, delay in care/vaccines as well as having to wait until open enrollment to add them.

HMO Plan Requirements

  • HMOs require the selection of a PCP.

  • Plans will often “auto assign” a Doctor in the area.

  • You must contact your plan PRIOR to your first visit with Positively Pediatrics and have Dr. Purman listed as the PCP.

  • Failure to do this will lead to large, unnecessary bills that become your responsibility.

  • Information listed below will help to make this process easier for you.

  • HMO plans require a referral from our office PRIOR to the specialist appointment.

  • For this to be completed, the child MUST be seen in our office to discuss the concern for us to process this referral.

  • Once this is completed, the parent will need to contact us directly with the appointment date, time, office and provider so that we may log in to your health care plan to enter this information.
